Acrylic Keychain Order Form

I've made a quick Google Docs order form if anyone would like custom items before I make a listing on Etsy.  This has my current stock of acrylic blanks.  I'll be adding what color choices are available and what type of hardware.  After filling out the order form, you will be contacted by me to confirm your order and you will receive a PayPal invoice.

Current pricing:
2 color 3" key chain - $8.25
2 color 1.5" charm -  $4
Add monogram/name - $1
Add double sided design - $3 (key chain)/$1.5 (charm)

Custom shape requests subject to minimum qty orders.  Must inquire.
